Investigation found the clinic's spirometer had been replaced with a non-attested device and traces were synthesized. Reversal cancels 14 mints totaling 25.7 HCR. The 9 affected wallets were notified; 7 have positive balances absorbing the reversal cleanly; 2 went negative and are under separate settlement.

Adopted v3.4 client across all 1 validators. Adds metering for HCC contract pricing computations to prevent denial-of-service via pathological cohort definitions. Backward compatible; no chain reset required.

Reversal time-bound · 365 days from original event

Codified that REVERSAL transactions must be filed within 365 days of the original event. After that, the historical record is closed; fraud findings can still be appended as annotations but cannot mint counter-debits.

BFT threshold review · 7-of-11 retained

Triennial review of consensus threshold. Council voted to retain 7-of-11 (rather than move to 8-of-11 for higher safety or 6-of-11 for higher liveness). Rationale: 7-of-11 tolerates 4 byzantine + the operator seat without halting; safer thresholds penalize legitimate validator outages.

Event class addition · HCC contracts & mints

Approved addition of two new event classes (HCC contract, HCC mint) with the launch of coin.healthcare. Distinct event-type IDs prevent conflation with HCR mints.

Unanimous.

Operator seat policy · cap at 1-of-11 weight

Codified that the operator (Conceptual Healthcare Corp.) holds exactly one validator seat at all times — never zero (so operator writes are traceable through the same threshold), never more than one (so operator cannot finalize alone or in coalition with a small subset).
